What is an Ambu bag/manual resuscitator?

An Ambu bag or manual resuscitator is a hand-held device commonly used to provide positive-pressure ventilation to patients who are not breathing or not breathing adequately. An Ambu bag/manual resuscitator combines a non-rebreathing valve mechanism, a self-inflating bag and oxygen reservoir into a single unit that may be attached to a face mask or endotracheal tube for manual ventilation of a patient. Invention of the Ambu bag has made possible successful resuscitation of patients during emergency situations. Prior to the invention of the manual resuscitator, efforts like mouth-to-mouth resuscitation were used for emergency ventilation. However, such methods caused problems like abdominal distention, vomiting, aspiration and poor ventilation carried a poor prognosis.

The indications for the use of the Ambu bag include respiratory failure, cardiopulmonary resuscitation and during conscious sedation. It is advised to practise relative caution while using the manual resuscitator in cases where the patient is suffering severe facial trauma or when open eye surgeries are performed. The use of an Ambu bag or Bag-Valve Mask ventilation is absolutely contraindicated in the presence of complete upper airway obstruction. Complications that may arise with the use of a manual resuscitator include aspiration, hyperventilation or hypoventilation. Ventilation through an Ambu bag requires a good seal and a patent airway. Practice with this important skill increases the clinician’s ability to provide effective ventilation. A variety of different types of Ambu bags are available for BVM ventilation purposes. Newer bags are equipped with a pressure valve. Some bags have one-way expiratory valves to prevent the entry of room air. Such bags allow for delivery of greater than 90% oxygen to ventilated and spontaneously breathing patients. Bags lacking this feature deliver a high concentration of oxygen during positive pressure ventilation but only deliver 30% oxygen during spontaneous breaths.
